HEALTH REPORT VERIFCATION

Students who have their health report from their home country must have the original one verified by Shaanxi Entry-Exit Inspection and Quarantine Bureau with a photo. If eligible, International Travel Healthcare Center will issue the Certificate of Health Examination Report for Foreigners.

If not eligible, supplement of examination or once more examination have to be taken. After examination, the Certificate of Health Examination Report for Foreigners will also be issued. Details below:

Documents: four 2-inch photos with back ground/ Passport copy/ original Foreigner Physical Examination Form (if available)

Fee: about 400 RMB

Route: Get on the Bus 313 from Stop of Jiaoda South Gate to Stop of Hanguang Road. The place is at the south of Hanguang Road. (It may take you about 15 RMB by taxi.)

You are NOT allowed to eat breakfast before physical examination.Please collect the verification result according to the date on the receipt you get after examination. Show the result to your instructor and prepare for visa documents.
